If you are a KCHP Member and haven't registered for member site access, please fill out the form below to request a personalized login.  Your login will be based on your email address, and you may request a personalized password.  Please note: any passwords deemed insecure will be rejected and modified by the system administrator.  Also, your ability to login must be approved and activated by KCHP staff.  You will receive email notification once your login ability goes live.
